新生児感染症は、胎児期の発育中または生後4週間以内に新生児が感染する病気です。 [ 1 ]新生児感染症は、母子感染、分娩時の産道感染、または出生後に感染する可能性があります。 [ 2 ]新生児感染症は、分娩後すぐに発症する場合もあれば、症状が現れるまでに数週間かかる場合もあります。HIV、B型肝炎、マラリアなどの一部の新生児感染症は、かなり後になってから初めて明らかになることがあります。感染の兆候や症状には、呼吸困難、体温不安定、易刺激性、哺乳不良、発育不全、泣き止まない、皮膚の発疹などがあります。
リスク要因には、母親の過去の感染、早産(妊娠37週未満)、および前期破水(羊膜の破裂)があり、これらは乳児の出生前に細菌が子宮内に侵入する経路となるため、新生児敗血症のリスクを大幅に高めます。早産児や低出生体重児は、新生児感染症にかかりやすいです。早産児は特にリスクが高いですが、すべての新生児が感染症を発症する可能性があります。分娩中の感染症に対する母親のスクリーニングは、新生児感染症のリスクを軽減します。妊婦は、新生児感染症予防のために分娩中に抗菌薬予防投与を受けることがあります。[ 3 ]
乳児呼吸窮迫症候群は、新生児感染症の一般的な合併症であり、早産児の呼吸困難を引き起こす状態です。呼吸窮迫症候群は新生児感染症の後に発生する可能性があり、この症候群は長期的に悪影響を及ぼす可能性があります。場合によっては、新生児呼吸器疾患は、将来の呼吸器感染症や肺疾患に関連する炎症反応に対する感受性を高める可能性があります。[ 4 ]
Antibiotics can be effective for neonatal infections, especially when the pathogen is quickly identified. Instead of relying solely on culturing techniques, pathogen identification has improved substantially with advancing technology; however, neonate mortality reduction has not kept pace. In industrialized countries, treatment for neonatal infections takes place in the neonatal intensive care unit (NICU). Neonatal infection can be distressing to the family and it initiates concentrated effort to treat it by clinicians. Research to improve treatment of infections and prophylactic treatment of the mother to avoid infections of the infant is ongoing.[5]
Signs and symptoms of infection include respiratory distress, temperature instability, irritability, failure to thrive, and skin rashes.
Early-onset sepsis (EOS), defined as onset of symptoms within 72 hours of life, is generally caused by transmission of pathogens from the female genitourinary system to the fetus. Pathogens can infect the fetus via vertical transmission (direct transmission through the placenta in utero) or infection during delivery as fetus passes through vaginal canal.
Late-onset sepsis (LOS), defined as onset of symptoms after 72 hours of life, is generally caused by transmission of pathogens from the environment after delivery. Infants requiring intravascular catheters and other invasive procedures are at increased risk for developing LOS.[3]
There are many etiologies of neonatal infection, including bacterial, viral and fungal pathogens. The source of infectious bacteria and other pathogens is often the maternalgastrointestinal and genitourinary tract. Many of the maternal infections with these organisms are asymptomatic in the mother. Other maternal infections that may be transmitted to the infant in utero or during birth are bacterial and viral sexually transmitted infections. The infant's ability to resist infection is limited by its immature immune system. In addition, the immune system of the neonate may respond in ways that can create problems that complicate treatment, such as the release of inflammatory chemicals. Congenital defects of the immune system also affect the infants ability to fight off the infection.[6]

グループB連鎖球菌(GBS)、別名Streptococcus agalactiaeは、新生児の早期発症感染症の大部分の原因として一般的に特定されている細菌です。GBSは、妊婦の消化管および生殖器に定着する被膜を持つグラム陽性球菌です。母体の感染は通常無症状です。この病原体は垂直感染します(分娩開始後、母親の膣から胎児の羊水に直接感染します)。GBSの有病率が高いため、妊娠中に細菌のルーチン検査が行われます。細菌が母体の消化管/泌尿生殖器で見つかった場合、母親はIV抗生物質(通常はペニシリンまたはアンピシリン)を投与されます。[ 8 ]
大腸菌は、妊娠患者の消化管および泌尿生殖器系に高頻度で存在するため、新生児感染症を引き起こす可能性のある莢膜を有するグラム陰性桿菌です。B群レンサ球菌感染症の予防が進歩するにつれて、β-ラクタム耐性大腸菌感染症が極低出生体重児や未熟児の新生児死亡の原因として増加しています。新生児大腸菌感染症の一般的な合併症には、新生児敗血症や新生児髄膜炎などがあります。 [ 9 ]
淋菌は一般的な性感染症であり、分娩時に妊婦に存在する可能性があります。この病原体は通常分娩中に感染し、母親の感染が判明している症例の 30 ~ 40% で発生します。さらに、未治療の母親の淋病は早産のリスクを高める可能性があります。新生児における淋菌感染症の最も一般的な症状は新生児結膜炎であり、緑黄色の滲出物とまぶたの腫れを伴う眼の感染症です。治療しないと、この感染症は永久的な視力障害につながる可能性があります。淋菌結膜炎の治療は、セフトリアキソン(抗生物質)の単回投与です。通常、すべての新生児(症状や危険因子に関係なく)は、分娩後に両眼にエリスロマイシン軟膏を塗布されます[ 10 ] 。
リステリア・モノサイトゲネスは、汚染された食品から感染し、母親に存在するグラム陽性桿菌です、母親に胃腸疾患として現れる症状によって判断されることがあります。母親は、ホットドッグ、未殺菌牛乳、デリカテッセンの肉、チーズなどの動物性食品を含む食品を摂取することで感染します。 [ 11 ]
クロストリジウム・テタニは新生児に全身性破傷風を引き起こす可能性があります。これは通常、母親が破傷風のワクチン接種を受けておらず、赤ちゃんが受動免疫を獲得していない場合に発生します。臍帯部が最も感染しやすい部位です。 [ 12 ]
あまり一般的ではない細菌性病原体には、化膿レンサ球菌、緑色レンサ球菌、肺炎レンサ球菌、インフルエンザ菌、緑膿菌などがある。[ 13 ]
ヒト免疫不全ウイルス(HIV)感染は、新生児の分娩時、母子感染による胎内感染、または授乳による出生後感染のいずれかで起こり得ます。感染の大部分は分娩時に起こります。感染は複数のリスク要因に依存しますが、通常は母親のHIVウイルス量に左右されます。HIV感染を減らすための戦略には、以下のようなものがあります。
Symptoms of HIV in a child will vary depending on the age of presentation. Common symptoms include failure to thrive, recurrent infections such as pneumonia, intermittent diarrhea, swollen lymph nodes and oral thrush. In infants, diagnostic testing for HIV relies of detection of the virus in the bloodstream. For infants born to HIV-infected mothers, diagnostic testing will be performed within days of delivery, at 1–2 months and at 4–6 months of age.[15]
Cytomegalovirus (CMV) is the most common congenital viral infection, usually transmitted through the placenta during pregnancy. Most neonates with congenital CMV infection will not have any symptoms, but a minority of infected newborns will have symptomatic infection. Common symptoms include rash, microcephaly (small head), low birth weight, jaundice, thrombocytopenia, seizures and retinitis. Long-term complications of congenital CMV infections may include sensorineural hearing loss, developmental delay, and seizures. Due to high prevalence of disease, CMV is not routinely screened in pregnant patients.[16]
Herpes simplex virus (HSV), which commonly causes cold sores and painful genital blisters can cause congenital infection via direct contact with genital tract lesions during delivery. Neonatal HSV may be classified into three categories based on symptom presentation:
Maternal infection with rubella virus during pregnancy can lead to congenital rubella syndrome. The risk of congenital infection is highest during the first trimester (< 12 weeks). Risk of congenital rubella is increased among immigrant women from countries without adequate vaccination programs. Common symptoms include cataracts, hearing impairment, developmental delay and congenital heart disease.[18]
Zika virus is an arthropod-borne virus transmitted by mosquitos, and infection during pregnancy can lead to severe congenital abnormalities in a newborn. Congenital infection can lead to fetal growth restriction and CNS abnormalities, including microcephaly, ventriculomegaly and intracranial calcifications.[19]
There are five liver specific viruses (hepatitis A, B, C, D, E) that could potentially harm the mother and child. Acute hepatitis A virus or acute hepatitis E virus infection present the greatest risk to maternal and fetal health and increased risk of adverse pregnancy outcomes. Hepatitis B, C and D virus present a risk of mother to child transmission but are dependent on the severity of the underlying disease in the mother. However, hepatitis B virus is the major cause of neonatal infection.[20]
Other viral infections, such as respiratory syncytial virus (RSV), metapneumovirus (hMPV), rhinovirus, parainfluenza (PIV), and human coronavirus in the neonatal period are associated with recurrent wheezing in later childhood.
In very low birth weight infants (VLBWI), systemic fungus infection is a hospital-acquired infection with serious consequences. The pathogens are usually Candida albicans and Candida parapsilosis. A small percentage of fungal infections are caused by Aspergillus, Zygomycetes, Malassezia, and Trichosporon.[22][23] Infection is usually late-onset. Up to 9% of VLBWI with birth weights of <1,000 g develop these fungus infections leading to sepsis or meningitis. As many as one-third of these infants can die. Candidiasis is associated with retinopathy, prematurity and negative neurodevelopmental consequences. Candida can colonize the gastrointestinal tract of low birthweight infants (LBI). This gastrointestinal colonization is often a precursor to a more serious invasive infection. The risk of serious candida infection increases when multiple factors are present. These are: thrombocytopenia, the presence of candidal dermatitis, the use of systemic steroids, birth weights of <1,000 g, presence of a central catheter, postponing enteral feeding, vaginal delivery, and the amount of time broad-spectrum antibiotics were given.[23]

子宮内で原虫トキソプラズマ・ゴンディに感染した乳児は、脈絡網膜炎または眼トキソプラズマ症を発症して生まれることがあります。世界的に、眼底感染症の最も一般的な原因です(後眼部)。最も一般的な兆候は、片眼の視力低下です。その他の兆候や症状は新生児期以降に現れることがあり、後年の脈絡網膜炎の発症、頭蓋内石灰化、水頭症、中枢神経系の異常などが含まれます。[ 24 ]
早産児は、敗血症や髄膜炎などの重篤な合併症を含む感染症のリスクが高い。早産児は通常、IgG抗体の減少や補体活性化の低下により、免疫系が十分に機能していない。さらに、早産児は入院期間が長く、感染リスクを高める侵襲的な医療機器の留置が必要となる場合もある。
新生児感染症の母親側の危険因子には以下が含まれる。

Inflammation accompanies infection and is likely to complicate treatment and recovery. Inflammation is linked to reduced growth of the lungs of the premature baby.[26]
The recent identification of the presence of microorganisms in maternal-infant body fluids that were previously thought to be sterile has provided one explanation for the presence of the inflammatory response in both the mother and infant. Sixty-one percent of pregnant women with chorioamnionitis, or inflammation of the amniotic fluid, were found to be infected by microorganisms. Often, more than one pathogen was present. In fifteen percent of pregnant women inflammation was still evident even though there was no evidence of pathogens. This may indicate that there are other causes. A high percentage, 51% to 62%, of pregnant women who had chorioamnionitis also had inflammation of the placenta.[26]

Diagnosis of infection is based upon the recovery of the pathogen or pathogens from the typically sterile sites in the mother or the baby. Unfortunately, as many half of pregnant women are asymptomatic with a gonorrhea infection and other sexually transmitted infections.[27][28][29] Samples are obtained from urine, blood or cerebrospinal fluid. Diagnosis of infection can also be aided by the use of more nonspecific tests such as determining the total white blood cell count, cytokine levels and other blood tests and signs.[30]
Symptoms and the isolation of the virus pathogen the upper respiratory tract is diagnostic. Virus identification is specific immunologic methods and PCR. The presence of the virus can be rapidly confirmed by the detection of the virus antigen. The methods and materials used for identifying the RSV virus has a specificity and sensitivity approaching 85% to 95%. Not all studies confirm this sensitivity. Antigen detection has comparatively lower sensitivity rates that approach 65% to 75%.[35]
Congenital malaria has its own set of signs:

新生児の感染を減らすため、英国と米国では妊婦に対するHIV、B型肝炎、梅毒のスクリーニング検査が利用可能である。[ 37 ] [ 38 ]
出産前に膣内抗生物質洗浄剤で治療しても、B群レンサ球菌(GBS)感染は予防できない。[ 31 ] [ 39 ]出産前に膣内クロルヘキシジンで治療しても、新生児感染は予防できない。[ 40 ]
GBS菌は女性の30%の下部生殖器に定着する可能性があるため、通常、妊婦は妊娠35週から37週の間にこの病原体の検査を受けます。出産前に母親を抗生物質で治療すると、新生児の感染率が低下します。[ 31 ]赤ちゃんの感染予防は、母親をペニシリンで治療することによって行われます。この予防的治療が採用されて以来、GBS感染による乳児死亡率は80%減少しました。[ 36 ]
症状のある性器ヘルペスがあり、抗ウイルス予防療法を受けている母親は、出産時に活動性の症状のある症例になる可能性が低く、出産時にHSVを伝染させるリスクを減らすことができる可能性があります。帝王切開は乳児の感染リスクを減らします。[ 41 ]
母乳育児は新生児をいくつかの感染症から守ることが示されている。[ 42 ] [ 43 ] [ 44 ] [ 45 ] [ 46 ] 母乳は壊死性腸炎から守る。[ 47 ]
A型肝炎、B型肝炎、D型肝炎はワクチン接種で予防できます。A型肝炎はHAVに対する免疫接種で予防でき、B型肝炎とD型肝炎はどちらもHBVに対する免疫接種で予防できます。[ 20 ]
B型肝炎ワクチンは、新生児が最初に受ける2つの注射のうちの1つです。[ 48 ]もう1つは、ビタミンK欠乏性出血を予防するためのビタミンKです。[ 49 ]妊婦がB型肝炎陽性の場合、母子感染を防ぐために、B型肝炎ワクチンに加えて、赤ちゃんにB型肝炎免疫グロブリンの追加投与が行われます。[ 50 ]現在、B型肝炎の治療法がないため、予防が非常に重要です。[ 51 ]
一般的に、ワクチン未接種者では、ウイルス性肝炎ウイルスは細胞傷害性リンパ球を介した細胞性免疫応答により肝臓に損傷を与えます。[ 52 ]急性ウイルス性肝炎は、黄疸、ビリルビン値の上昇、肝酵素(アスパラギン酸アミノトランスフェラーゼ、アラニンアミノトランスフェラーゼ、アルカリホスファターゼ)値の上昇、およびインフルエンザ様症状を引き起こす可能性があります。[ 52 ]重要なことに、新生児の急性HBV感染は、約90%の確率で慢性B型肝炎に進行します。[ 53 ] B型肝炎とD型肝炎の両方において、最も重大な長期合併症は肝硬変と肝細胞癌への進行であり、高い罹患率と死亡率を伴います。[ 54 ]
新生児感染症の治療は、通常、原因の診断が確定する前に開始されます。新生児感染症は、抗生物質で予防的に治療することができます。[ 55 ]母親への抗生物質投与は、主にB群レンサ球菌から保護するために使用されます。[ 30 ]
性器ヘルペスの既往歴のある女性は、出生時に乳児に感染する可能性のある症状のある病変やウイルスの排出を防ぐために抗ウイルス薬で治療することができます。使用される抗ウイルス薬には、アシクロビル、ペンシクロビル、バラシクロビル、ファムシクロビルなどがあります。胎児ではごく少量の薬剤しか検出されません。アシクロビルに起因する乳児の薬剤関連異常の増加はありません。抗ウイルス薬の長期的な影響は、子供の成長と発達後の影響については評価されていません。好中球減少症は、新生児HSV感染症のアシクロビル治療の合併症となる可能性がありますが、通常は一過性です。[ 41 ]免疫グロブリン療法による治療は有効性が証明されておらず、推奨されていません。[ 56 ]
毎年最大 330 万人の新生児が死亡し、そのうち 23.4% は新生児感染症で死亡しています。敗血症または肺炎による死亡の約半分は、産後最初の 1 週間に発生します。先進国では、B 群連鎖球菌が確認された母親への予防的抗生物質治療、新生児の敗血症の早期発見、および新生児への抗生物質の投与により、死亡率が低下しています。[ 31 ]北米における新生児ヘルペスは、 出生 10 万人あたり 5 ~ 80 人であると推定されています。HSV は、米国以外の母親では有病率が低くなっています。英国では発生率ははるかに低く、出生 10 万人あたり 1.6 人であると推定されています。感染した乳児の約 70% ~ 80% は、HSV 感染の既往歴がない母親から生まれています。[ 41 ]
新生児死亡率が低い地域には、ヨーロッパ、西太平洋、アメリカ大陸などがあり、これらの地域では敗血症による新生児死亡率は世界全体の新生児死亡の9.1%から15.3%を占めています。これは、ナイジェリア、コンゴ民主共和国、インド、パキスタン、中国などの資源の乏しい国々における全死亡の22.5%から27.2%という割合とは対照的です。[ 31 ]
英国では、B型肝炎、梅毒、HIVの新規スクリーニングで陽性となった妊婦の割合は、2010年以降、それぞれ約0.4%、0.14%、0.15%で一定している。過去の診断を含めた妊婦におけるB型肝炎とHIVの推定有病率は、それぞれ0.67%と0.27%と高かった。抗体レベルが低いため風疹に感受性があると評価された妊婦は60%以上増加し、約7.2%になった。しかし、この増加はおそらく検査方法と評価基準の変更によるものと考えられる。[ 57 ]
北米では、1950年代以前は、A群β溶血性連鎖球菌(GAS)が新生児敗血症に関連する最も一般的な病原体でした。過去20年間で、敗血症を引き起こす最も一般的な病原体は、感染した中心静脈カテーテルまたは動脈カテーテルに関連するバイオフィルムとして存在するコアグラーゼ陰性ブドウ球菌です。[ 55 ]感染症は致命的になる可能性があり、小児期まで生き延びた乳児の長期的な罹患率と障害につながります。[ 55 ]新生児敗血症は、出生1000人あたり128例に影響を及ぼします。敗血症の乳児に髄膜炎が発生することがあります。[ 30 ] HSVに感染している妊婦は、妊娠中に少なくとも1回の再燃を起こす可能性が75%あります。[ 41 ]限られた研究では、マラリアに感染した母親から生まれたアフリカの乳児は、7%が先天性マラリアを発症することがわかった。[ 22 ]
早期発症敗血症は生後1週間以内に発生する可能性があります。通常、出生後1日目に明らかになります。このタイプの感染症は通常、乳児の出生前に感染します。前期破水やその他の産科的合併症は、早期発症敗血症のリスクを高める可能性があります。羊膜が分娩の18時間以上前に破裂した場合、乳児はこの合併症のリスクが高くなります。早産、低出生体重、絨毛膜羊膜炎、母体の尿路感染症および/または母体の発熱は、早期発症敗血症のリスクを高める合併症です。早期発症敗血症は、重篤な呼吸器症状によって示されます。乳児は通常、肺炎、低体温、またはショックを発症します。死亡率は30~50%です。[ 30 ]
生後1週間以降、30日未満に発生する感染症は、後期発症感染症とみなされます。産科的および母体の合併症は、通常、これらの後期発症感染症の原因ではありません。これらは通常、新生児集中治療室で乳児が感染します。新生児集中治療室での広域スペクトル抗生物質の広範な使用は、侵襲性抗生物質耐性菌の蔓延率を高める可能性があります。[ 30 ]胎便吸引症候群の死亡率は4%強です。これは、すべての新生児死亡の2%を占めています。[ 58 ]
感染リスクに対する感受性と免疫不全は、活発な研究分野である。新生児感染症におけるウイルスの役割に関する研究は不足している。腸、皮膚、その他のヒトマイクロバイオームの役割と保護効果、および新生児期の定着に関する研究も継続されている。[ 26 ] [ 30 ]資源が豊富な国と貧しい国を比較すると、診断の成功を比較することは困難である。工業化された地域では、臨床検査室で診断と病原体の存在を確認できるからである。臨床検査はすべての環境で利用できるとは限らず、臨床医は新生児の感染の兆候に頼らざるを得ない。アフリカと東南アジアからの研究データは少ない。[ 31 ]
いくつかの研究の結果、資源の乏しい地域でB群レンサ球菌感染症の母親を特定できる診断ツールと手順が特定されました。これらの手順は簡単かつ安価に使用できます。感染が確認された母親は、赤ちゃんの出産前に予防的に治療を受けることができます。[ 31 ]
ラクトバチルス属のプロバイオティクス投与は一定の成果を上げている。[ 59 ]
GBSワクチンは現在試験中ですが、現時点では入手できません。ワクチン接種により、米国では早産児のGBS感染の4%、新生児のGBS感染の60~70%を予防できると推定されています。母親へのワクチン接種による予測される効果は、乳児におけるGBS疾患899例と死亡35例の予防です。GBS予防による費用削減額は4300万ドルを超える可能性があります。ワクチン接種は、スクリーニングや予防的治療が不可能な低~中所得国で特に有益となる可能性があります。アナリストは、GBSワクチン接種により乳児GBS症例の30~54%を予防できると予測しています。スクリーニング、予防的抗生物質、およびワクチン接種により、感染の48%を予防できます。[ 60 ]
妊婦とその新生児は、リステリア菌感染症(リステリア症)にかかる可能性がはるかに高い。
、滅菌されていない方法で臍帯を切断したり、臍帯の断端に滅菌されていない伝統的な治療法を適用したりすることによって最も多く発生しますが、臍帯の断端の感染は必ずしも明らかではありません。不潔な手で分娩を行ったり、汚染された表面で分娩を行ったりすることも、母体および新生児破傷風(MNT)のリスク要因です。
