それぞれの管轄区域では、検査対象者、検査頻度、分析プロトコル、サンプル採取、検査結果の利用など、さまざまな検査プロトコルを採用している。[6] [7] [8]このばらつきは、症例数や検査数、症例死亡率、症例人口統計など、報告された統計に大きな影響を与えている可能性が高い。[9] [10] [11] [12] SARS-CoV-2の感染は曝露後数日(症状発現前)に起こるため、頻繁な監視と迅速な結果の提供が緊急に必要とされている。[13]
検査分析は、多くの場合、自動化された高スループットの医療研究室で臨床検査技師によって行われます。迅速な自己検査やポイントオブケア検査も利用可能で、精度は低くなりますが、より迅速かつ安価な方法でウイルスを検査できます。[14] [15]
方法

ウイルス検査が陽性であれば現在感染していることを示し、抗体検査が陽性であれば過去に感染していたことを示します。[17]その他の検査方法としては、 CTスキャン、体温上昇の確認、血中酸素濃度の低下の確認、訓練された犬による検出などがあります。[18] [19] [20]
ウイルスの検出
ウイルスの検出は通常、ウイルスの内部RNA、またはウイルスの外側にあるタンパク質片を調べることによって行われます。ウイルス抗原(ウイルスの一部)を調べる検査は抗原検査と呼ばれます。
ウイルスのRNAの存在を検出することでウイルスを探す検査には複数の種類があります。これらは分子生物学にちなんで核酸検査または分子検査と呼ばれています。2021年現在、最も一般的な分子検査は逆転写ポリメラーゼ連鎖反応(RT-PCR)検査です。[21] 分子検査で使用される他の方法には、CRISPR、等温核酸増幅、デジタルポリメラーゼ連鎖反応、マイクロアレイ分析、次世代シーケンシングなどがあります。[21][update]
逆転写ポリメラーゼ連鎖反応(RT-PCR)検査
ポリメラーゼ連鎖反応(PCR)は、 DNAの小さな特定のセグメントを数十万回増幅(複製)し、分析に十分な量を作成するプロセスです。テストサンプルは、 DNAを抽出できる特定の化学物質[22] [23]で処理されます。逆転写によりRNAがDNAに変換されます。
逆転写ポリメラーゼ連鎖反応(RT-PCR)は、まず逆転写を利用してDNAを取得し、続いてPCRでそのDNAを増幅して分析に十分な量を生成します。[23] RT-PCRはRNAのみを含むSARS-CoV-2を検出できます。RT-PCRプロセスには通常数時間かかります。[24]これらの検査は分子検査または遺伝子検査とも呼ばれます。[3]
リアルタイムPCR(qPCR)[25]は自動化、高スループット、より信頼性の高い機器などの利点があり、好まれる方法となっている。[26] [27]
この複合技術はリアルタイムRT-PCR [28]または定量的RT-PCR [29]と呼ばれ、 qRT-PCR [30]、rRT-PCR [31]、またはRT-qPCR [32]と略されることもありますが、RT-PCRまたはPCRが使用されることもあります。定量的リアルタイムPCR実験の出版のための最小情報(MIQE)ガイドラインでは、 RT-qPCRという用語を提案していますが、[25]すべての著者がこれに従っているわけではありません。
迅速分子検査の平均感度はブランドによって異なります。ID NOWの場合、平均感度は73.0%、平均特異度は99.7%でした。Xpert Xpressの場合、平均感度は100%、平均特異度は97.2%でした。[33] [34]
診断テストでは、感度はテストが真陽性をどれだけ正確に識別できるかの尺度であり、特異度はテストが真陰性をどれだけ正確に識別できるかの尺度です。診断テストとスクリーニングテストの両方において、通常、感度と特異度の間にはトレードオフがあり、感度が高いほど特異度は低くなり、逆もまた同様です。

90% の特異度検査では、感染していない人の 90% を正しく特定し、10% は偽陽性の結果になります。
サンプルは、鼻咽頭スワブ、痰(咳き出した物質)[35]、[35]咽頭スワブ、[36]吸引カテーテルで採取した深部気道物質[36]または唾液[37] など、さまざまな方法で採取できます。[ 38] ドロステンらは、2003年のSARSについて、「診断の観点から、鼻と咽頭スワブは診断にあまり適していないことに留意することが重要です。これらの物質には痰よりもウイルスRNAがかなり少なく、これらの物質のみを検査するとウイルスが検出されない可能性があるためです。」と述べています。[39]
RT-PCRによる臨床検体の感度は、鼻腔スワブで63%、咽頭スワブで32%、糞便で48%、痰で72~75%、気管支肺胞洗浄液で93~95%である。[40]
ウイルスを検出する可能性は、採取方法と感染後の経過時間によって異なります。ドロステン氏によると、咽頭スワブによる検査は最初の1週間のみ信頼できます。その後、ウイルスは咽頭を離れ、肺で増殖する可能性があります。2週間目は、痰または深部気道の採取が推奨されます。[36]
唾液採取は鼻や喉のスワブと同じくらい効果的かもしれないが[37]、確実ではない。[41] [38]唾液を採取することで、医療従事者との密接な接触がなくなるため、リスクが軽減される可能性がある。[42]患者にとっても快適である。[43]隔離された人は自分でサンプルを採取できる。[42]唾液検査の診断価値は、サンプル採取部位(喉の奥、口腔、唾液腺)によって異なる。[38]いくつかの研究では、唾液はスワブサンプルと比較して感度と一貫性が高いことがわかっている。[44] [45] [46]
2020年8月15日、米国FDAは、イェール大学で開発された、数時間で結果が出る唾液検査の緊急使用許可を与えた。[47] [48]
2021年1月4日、米国FDAは、 Curative SARS-Cov-2 AssayリアルタイムRT-PCR検査における偽結果、特に偽陰性結果のリスクについて警告を発した。 [49]
上気道検体で測定されたウイルス量は、症状発現後に減少する。[50]回復後、多くの患者では上気道検体でウイルスRNAが検出されなくなる。検出可能な患者でも、回復後3日目のRNA濃度は、複製能のあるウイルスが確実に分離された範囲を下回ることが多い。[51]病気の期間と回復後の上気道検体におけるウイルスRNAの排出期間との間には明確な相関関係は示されていない。[52]
その他の分子検査
等温核酸増幅検査でもウイルスのゲノムを増幅します。加熱と冷却の繰り返しサイクルが不要なため、PCR よりも高速です。これらの検査では通常、蛍光タグを使用して DNA を検出し、専用の機械で読み取ります。[引用が必要]

抗原検査

抗原は、免疫反応を引き起こす病原体の一部です。抗原検査では、ウイルス表面の抗原タンパク質を探します。コロナウイルスの場合、これらは通常、表面スパイクのタンパク質です。[56] SARS-CoV-2抗原は、COVID-19の症状が現れる前に(SARS-CoV-2ウイルス粒子としてすぐに)より迅速な検査結果で検出できますが、ウイルスのPCR検査よりも感度は低くなります。[57]
COVID-19迅速抗原検査は、現在のウイルス感染を示す特定のウイルス抗原の存在を検出するラテラルフロー 免疫測定法です。抗原検査はすぐに(約15~30分以内に)結果が得られ、ほとんどがポイントオブケアまたはセルフテストとして使用できます。セルフテストは、自宅やどこでも受けることができ、使いやすく、迅速に結果が得られる迅速検査です。[58]抗原検査は、鼻咽頭、鼻腔スワブ、または唾液検体で行うことができます。[15]
SARS-CoV-2を識別できる抗原検査は、ウイルスを検査するためのより迅速で安価な方法を提供します。[14]抗原検査は、一般的に、リアルタイム逆転写ポリメラーゼ連鎖反応(RT-PCR)やその他の核酸増幅検査(NAAT)よりも感度が低いです。[15]
抗原検査は、検査をはるかに大規模にスケールアップする方法の1つかもしれない。[56]等温核酸増幅検査は、機械1台につき一度に1つのサンプルしか処理できない。RT -PCR検査は正確だが、検査を実行するには時間がかかり、エネルギーと訓練された人員が多すぎる。[56]「[PCR]検査で1日3億件の検査を行うことや、仕事や学校に行く前に全員を検査することは決してできないだろう」と、ホワイトハウスコロナウイルスタスクフォースの責任者であるデボラ・バークスは2020年4月17日に述べた。「しかし、抗原検査なら可能かもしれない」[59]

呼吸器ウイルスのスワブには、検出できるだけの抗原物質が不足していることが多い。[61]これは特に、鼻水がほとんどないかまったくない無症状の患者に当てはまる。抗原検査ではウイルスタンパク質は増幅されない。[56] [62] 16種類の抗原検査の有効性を調べた64の研究に基づくコクランレビューでは、症状のある人の平均72%でCOVID-19感染を正しく特定したのに対し、症状のない人では58%だったと判定された。[63] [更新が必要]症状が最初に現れてから最初の1週間に検査を使用した場合、最も正確(78%)だったが、これは感染後最初の数日間に体内に最も多くのウイルスが存在するためと考えられる。[63] 抗原検査がCOVID-19に対して有効かどうか疑問視する科学者もいるが、[62]ウイルス量が多く伝染性がある場合には抗原検査の感度が非常に高いため、公衆衛生スクリーニングに適していると主張する科学者もいる。[64] [65]定期的な抗原検査は、無症状の人が感染しているかどうかを迅速に特定することができ、確定診断が必要な場合はフォローアップPCRを使用することができます。[66]
抗体検査
体はウイルス感染に反応して、ウイルスを中和するのに役立つ抗体を生成します。 [67]血液検査(血清学的検査または血清学的免疫測定法[3]とも呼ばれます)は、そのような抗体の存在を検出できます。[68]抗体検査は、人口のどの程度がかつて感染したかを評価するために使用でき、それを使用して病気の死亡率を計算することができます。[5]また、COVID-19治療のために回復期血漿1単位に含まれる抗体の量を特定したり、特定のワクチンが適切な免疫反応を生成するかどうかを確認したりするためにも使用できます。[69]
SARS-CoV-2抗体の効力と防御期間は確立されていない。[5] [70] そのため、抗体検査が陽性であっても、将来の感染に対する免疫があることを意味するわけではない。さらに、軽度または無症状の感染が検査で検出するのに十分な抗体を生成するかどうかは確立されていない。[71]一部の疾患に対する抗体は血流中に何年も残りますが、他の疾患に対する抗体は消えていきます。[56]
最も注目すべき抗体はIgMとIgGである。IgM抗体は一般に感染後数日で検出されるが、感染過程および感染後のレベルは十分に特徴付けられていない。[72] IgG抗体は一般に感染後10~14日で検出され、通常は感染後28日頃にピークを迎える。[73] [74]しかし、他の感染症で見られるこの抗体発達パターンはSARS-CoV-2には当てはまらないことが多く、IgMはIgGの後に発生することもあれば、IgGと一緒に発生することもあれば、まったく発生しないこともある。[75] ただし、一般的にIgMの検出中央値は発症後5日で、IgGは発症後14日で検出される。[76] IgGレベルは2~3ヶ月後に大幅に低下する。[77]
遺伝子検査は抗体検査よりも早く感染を確認します。遺伝子検査が陽性だった人のうち、感染7日目に抗体検査が陽性になったのはわずか30%でした。[71]
抗体検査の種類
迅速診断検査(RDT)
RDTは通常、医療現場で実行できる小型で持ち運び可能な陽性/陰性のラテラルフローアッセイを使用します。RDTは血液サンプル、唾液サンプル、または鼻腔スワブ液を処理します。RDTは陽性または陰性の結果を示すために色付きの線を生成します。[78]
酵素免疫測定法(ELISA)
ELISAは定性的または定量的であり、通常は研究室が必要です。これらのテストは通常、全血、血漿、または血清サンプルを使用します。プレートは、SARS-CoV-2スパイクタンパク質などのウイルスタンパク質でコーティングされています。サンプルはタンパク質とともにインキュベートされ、抗体が結合できるようにします。その後、抗体-タンパク質複合体は、色/蛍光読み取り値を生成する別の抗体の洗浄で検出できます。[78]
中和試験

化学発光免疫測定法
化学発光免疫測定法は定量的な検査です。血液、血漿、血清を採取します。サンプルは既知のウイルスタンパク質、緩衝試薬、および特定の酵素標識抗体と混合されます。その結果、発光します。化学発光微粒子免疫測定法では、磁性タンパク質コーティング微粒子を使用します。抗体はウイルスタンパク質に反応して複合体を形成します。二次酵素標識抗体が追加され、これらの複合体に結合します。結果として生じる化学反応により光が発生します。その放射輝度を使用して抗体の数を計算します。この検査では、IgG、IgM、IgAなど、複数の種類の抗体を識別できます。[78]
結合抗体に対する中和
大規模なCOVID-19抗体検査のほとんど、あるいはすべては、結合抗体のみを調べており、より重要な中和抗体(NAb)は測定していない。[79] [80] [81] NAbは、感受性細胞への付着や侵入をブロックすることでウイルス粒子の感染性を中和する抗体である。例えばSARS-CoV-2のようなエンベロープウイルスは、膜融合までの複製サイクルのステップをブロックすることで中和される。[82] [67]非中和抗体は、ウイルス表面の重要な構造に結合しないか、結合してもウイルス粒子に感染性を残す。抗体は依然として、免疫系によるウイルス粒子または感染細胞の破壊に寄与する可能性がある。[83] [67]マクロファージ上の受容体と相互作用することで感染性を高める可能性もある。[84]ほとんどのCOVID-19抗体検査では、結合抗体のみが検出された場合に陽性の結果が出るため、これらの検査では被験者が再感染を防ぐ保護NAbsを生成したことを示すことはできません。[80] [81]
結合抗体はNAbの存在を意味すると予想され[81]、多くのウイルス性疾患では総抗体反応はNAb反応とある程度相関している[85]が、これはCOVID-19では確立されていない。中国で軽度の症状を経験した回復患者175人を対象とした研究では、退院時または退院後にNAbが検出されなかった人が10人いたと報告されている。これらの患者がNAbの助けなしに回復した方法や、再感染のリスクがあったかどうかについては触れられていない[80]。さらに不確実な原因は、NAbが存在しても、HIVなどのウイルスはNAb反応を回避できることである。[79]
研究によると、元のSARSウイルス(現在のSARS-CoV-2の前身)に対するNAbsは2年間活性を維持し、6年後には消滅することが示されています。[86]しかし、メモリーB細胞やメモリーT細胞などのメモリー細胞[88]ははるかに長く持続し、再感染の重症度を軽減する能力がある可能性があります。[87]

嗅覚テスト
突然の嗅覚喪失は、COVID-19のスクリーニングに日常的に利用できる。国立衛生研究所の研究では、SARS-CoV-2に感染した人は、25%のエタノールと水の混合液の匂いを嗅ぐことができなかったことが示された。[89] 嗅覚喪失はさまざまな状態によって引き起こされる可能性があるため、嗅覚検査は決定的なものにはならず、PCR検査の必要性を示すものとなる。嗅覚喪失は他の症状よりも先に現れるため、広範囲にわたる嗅覚検査の実施が求められている。[90]嗅覚検査は迅速で簡単で、毎日自分で実施できるにもかかわらず、医療行政は一般的にこれを無視してきた。このため、一部の医学雑誌は嗅覚検査の採用を支持する論説を掲載している。[91]
イメージング
CTで最初に見られる典型的な特徴としては、周辺部または後部に分布する両側の多葉性のすりガラス陰影があります。 [92] COVID-19は、RT-PCRよりもCTを使用した方が精度よく特定できます。[93]
病気が進行するにつれて、胸膜下優位、クレイジーペービング、および硬化が進行する可能性があります。 [92] [94]胸部CTスキャンと胸部X線はCOVID-19の診断には推奨されません。COVID-19の放射線学的所見は特異性に欠けます。[92] [95]
胸部X線検査、コンピューター断層撮影、超音波検査はすべて、新型コロナウイルス感染症を検出できる方法です。

しかし、画像診断には欠点もある。CTスキャンに必要な機器はほとんどの病院では利用できないため、コロナウイルス感染症の検出に使用される他のツールほど効果的ではない。[96]パンデミックにおける困難な作業の1つは、各レポートを手動で検査することであり、これには多くの放射線科医と時間がかかる。[100]コロナウイルスの診断に胸部CTスキャンを使用する初期の研究では、いくつかの問題があった。これらの問題のいくつかには、重症の場合と入院している場合で病気の重症度特性が異なることが含まれていた。胸部CTスキャンを行うための基準は定義されていなかった。また、胸部CTスキャンの陽性結果の特徴付けもなかった。CTスキャンの所見は、コロナウイルスの陽性CTスキャン所見と同じではなかった。[99]一般的な臨床現場では、胸部画像診断はCOVID-19の定期的なスクリーニングには推奨されていない。無症状から軽度の症状の患者は、胸部CTスキャンによる検査は推奨されない。しかし、特に合併症や病気の進行を判断する際には、胸部画像診断を使用することが依然として重要です。また、COVIDの高リスク因子を持つ患者の場合、胸部画像診断が常に最初に行われるとは限りません。軽度の症状を示す高リスク患者の場合、胸部画像診断の所見は限られていました。CTスキャンはCOVID-19の診断に強力なツールですが、特異性が低く、胸部CTスキャンでCOVID-19と他のウイルス性肺炎を区別する際に放射線科医が経験する困難さのため、COVID-19を単独で特定するには不十分です。[98]
血清学(CoLabスコア)検査
緊急治療室で行われる標準的な血液検査(クイックスキャン)では、さまざまな値が測定されます。血液クイックスキャンを使用すると、コロナウイルスが血液にどのような変化を引き起こすかに基づいて開発されたアルゴリズムでCoLabスコアが計算されます。このソフトウェアは、緊急治療室で使用され、入院患者の病気の存在を迅速に排除することを目的としています。結果が陰性でない場合は、PCR(ポリメラーゼ連鎖反応)またはLAMP(ループ介在等温増幅)検査が行われます。[101]

動物
2021年5月、ロイター通信は、ワーゲニンゲン大学のオランダ人研究者らが、訓練されたミツバチが感染サンプル中のウイルスを数秒で検出できることを示し、検査施設が不足している国々に恩恵をもたらす可能性があると報じた。[102] パリのネッケル・コチン病院がフランス国立獣医学校と共同で行った2か月間の研究では、2021年5月に、犬による検査が現在のラテラルフロー検査よりも信頼性が高いことが報告された。[103]
2022年3月にパリの研究者らは、まだ査読されていないプレプリントで、訓練された犬が、症状の有無にかかわらず、人体におけるSARS-Cov2の存在を迅速に検出するのに非常に効果的であると報告した。犬は335人の汗のサンプルを嗅ぎ分け、そのうち症状のある78人と症状のない31人がPCR検査で陽性と判定された。犬は症状のある感染者の97%と無症状の感染者の100%を検出した。感染していないボランティアを特定する精度は91%、症状のない人の感染を除外する精度は94%だった。著者らは「犬による検査は非侵襲的で、即時かつ信頼性の高い結果が得られる。さらなる研究では、空港、港、鉄道駅、文化活動、スポーツイベントでの大規模な事前検査用の探知犬を評価するために、犬による直接の嗅覚に焦点を当てる予定だ」と述べた。[104] [105]

歴史

2020年1月、中国の科学者らは、SARS-CoV-2の最初の遺伝子配列をvirological.org [108]で公開した。virological.orgは「公衆衛生活動や研究を支援するために設計された公開前データのハブ」である。[109] 世界中の研究者らがそのデータを使い、ウイルスの分子検査を構築した。抗原と抗体に基づく検査は後に開発された。[要出典]

ドライブスルー検査

香港では、被験者は自宅にいて検体チューブを受け取ることができる。それに唾を吐き、返却すれば後で結果がわかる。[121]さらに、2023年秋までに、米国は全国の家庭に無料の在宅COVID-19検査を6回にわたって郵送した。迅速抗原検査はPCR検査ほど正確ではないが、分析のために検査結果を研究所に郵送する必要がなかった。[122] [123]
プールテスト
プール検査は、検査するサンプルをいくつか組み合わせることで、処理時間を短縮することができます。プールの結果が陰性であれば、すべてのサンプルが陰性です。検査結果が陽性であれば、サンプルを個別に検査する必要があります。[69]

多層テスト

必要量
必要な検査レベルは、病気の広がりに応じて異なります。感染者数が多いほど、感染拡大を抑えるために必要な検査数も増えます。COVID-19 は感染拡大の初期に指数関数的に増加する傾向があるため、必要な検査数も当初は指数関数的に増加します。適切に対象を絞った検査が感染者数よりも急速に増加すれば、封じ込めることができます。[要出典]

スナップショット大量テスト

人口の監視とスクリーニング

抗原検査

抗原検査は世界中で容易に利用可能であり、いくつかの保健規制当局によって承認されています。
Quidel社の「Sofia 2 SARS抗原FIA」[66] [163]は、モノクローナル抗体を使用してウイルスのヌクレオカプシド(N)タンパク質を検出するラテラルフローテストです。[164]結果は、同社のSofia 2デバイスで免疫蛍光法を使用して読み取られます。[164]このテストは核酸テストよりも簡単で安価ですが、精度は劣ります。実験室またはポイントオブケアで展開でき、15分で結果が得られます。[163]サンプルの抗原レベルが陽性であるが、テストの検出限界を下回る場合は偽陰性の結果が発生し、核酸テストでの確認が必要になります。[164]
Innova SARS-CoV-2抗原迅速定性検査は米国での使用が承認されていなかったが、同社によって販売されていた。FDAは2021年3月と4月にカリフォルニア州のInnova施設を査察し、中国で製造された検査の品質保証が不十分であることを発見した。[165] 2021年4月23日、同社はリコールを発表した。FDAは、臨床試験で見つかった偽陽性と偽陰性の割合がパッケージに記載されている割合よりも高かったため、消費者に機器を返品または破棄するよう警告した。[166]同社の10億回以上の検査が英国で配布され、ムーンショット作戦 の一環として30億ポンドの資金が投入されており、英国保健省は少なくとも2021年8月28日まで例外的な使用を認可している。[165] [165] 2020年の研究では、実験室の科学者が使用した場合、陽性症例の79%が見つかったが、一般市民が使用した場合、陽性症例は58%、リバプール市全体のスクリーニングに使用された場合、陽性症例は40%にとどまった。[167]
血清(抗体)検査
抗体は通常、感染開始から14日後に検出可能となる。複数の管轄区域で、これらの検査を用いて住民を調査している。[168] [169]この検査には血液サンプルが必要である。
Quest DiagnosticsやLabCorpなどの米国の民間研究所は、要請に応じて抗体検査を提供している。[170]

BMJの要約レビューでは、一部の「血清学的検査は、RT-PCR よりも安価で、医療現場での実施が容易である可能性がある」と指摘されており、そのような検査は以前に感染した個人を特定できるが、「血清学的検査を疫学的監視に使用する場合は注意が必要である」としている。レビューでは、「少なくとも 2 つの連続した検体に対して RT-PCR を実施し、可能であればウイルス培養も含める」という基準を参照して精度を評価する、より質の高い研究を求めている。[174] [175] CEBM の研究者は、病院内での「症例定義」で「CT 肺所見と関連する血液検査」を記録することを求めており[176]、WHO が「PCR の使用と解釈を標準化するプロトコル」を作成し、継続的に再調整することを求めている。[177]
正確さ
精度は特異性と選択性の観点から測定されます。検査エラーには偽陽性(検査結果は陽性だがウイルスが存在しない)または偽陰性(検査結果は陰性だがウイルスが存在している)があります。[179] 90万件以上の迅速抗原検査の研究では、偽陽性は0.05%、つまり2000年に1件の割合で発生することがわかりました。[180]
感度と特異度
感度は、検査がウイルスの存在を正確に特定できるかどうかを示します。各検査では、陽性結果を出すために最低限のウイルス量が必要です。感度90%の検査では、感染の90%を正確に特定しますが、残りの10%は見逃してしまいます(偽陰性)。比較的高い感度であっても、感染率の低い集団では偽陰性率が高くなることがあります。[179]
診断テストでは、感度はテストが真陽性をどれだけ正確に識別できるかの尺度であり、特異度はテストが真陰性をどれだけ正確に識別できるかの尺度です。診断テストとスクリーニングテストの両方において、通常、感度と特異度の間にはトレードオフがあり、感度が高いほど特異度は低くなり、逆もまた同様です。

90% の特異度検査では、感染していない人の 90% を正しく特定し、残りの 10% は偽陽性の結果となります。[引用が必要]
特異度の低い検査は、有病率が低い場合、陽性予測値(PPV) が低くなります。たとえば、発生率が 5% であるとします。特異度95 % の検査を使用して 100 人を無作為に検査すると、実際には陰性であるのに誤って陽性と判定される人が平均 5 人います。被験者の 5% が実際に陽性であるため、他の 5 人も正しく陽性と判定され、合計 10 人の陽性結果になります。したがって、PPV は 50% となり、[181]コイントスと変わりません。この状況では、2 回目の検査結果が最初の検査とは無関係であると仮定すると、最初の陽性結果の人を再検査すると、PPV は 94.5% に増加します。つまり、2 回目の検査で誤った結果が返されるのはわずか 4.5% であり、平均して誤った結果は 1 件未満になります。[182]
テストエラーの原因
感染の経過は、一部の検査の精度に影響します。サンプルは、ウイルスが定着する前に採取される場合もあれば、体がウイルスを排除し始めた後に採取される場合もあります。2020年5月のPCR-RT検査のレビューでは、偽陰性結果の中央値は、1日目の100%から4日目には67%に減少したことがわかりました。症状発現日の確率は38%でしたが、3日後には20%に減少しました。[183] [更新が必要]
PCR検査

RT-PCRは最も一般的に使用されている診断検査です。[184]鼻咽頭スワブによるPCR検査の感度は73%ですが、対照群とのPCR研究が不足しているため、特異性の体系的な分析は決定されていません。[185]
ある研究では、感度は1週目に最も高く(100%)、次いで症状発症から6週目までに89.3%、66.1%、32.1%、5.4%、そしてゼロとなった。[186] [検証に失敗した] [187]
感度はPCRサイクル数、サンプル採取から分析までの時間と温度にも左右される。[188]感染力の高い人の場合、20サイクルのサイクル閾値があればSARS-Cov-2を検出するのに十分である。[188]サイクル閾値が34を超えると、バイオセーフティレベルの高い施設以外では偽陽性となる可能性が高まっている。[188]
2020年7月、米国国立衛生研究所のアンソニー・ファウチ博士は、 35サイクル以上で実行されたRT-PCR検査から得られた陽性結果は、ほぼ常に「単なる死んだヌクレオチド」であると示唆した。 [189] 2020年8月には、「マサチューセッツ州、ニューヨーク州、ネバダ州の当局者がまとめた、サイクル閾値を含む3セットの検査データでは、ほとんどの検査が40 [サイクル] に制限を設定し、少数は37に設定していた」と報告され、CDCは「政策決定のために」サイクル閾値測定の使用を検討しているとのことだ。 [190] 2021年7月21日、CDCは「リアルタイムRT-PCR診断パン:使用説明書」の中で、検査結果は40サイクルで判定されるべきであると示唆した。[191]
オランダCDC主導の研究室調査では、7つのPCRキットを比較した。[192] BGI、R-Biopharm AG、BGI、KH Medical、Seegene製の検査キットは高い感度を示した。[193]
症状のない人を診断するには高感度キットが推奨されるが、症状のある患者を診断する場合は低感度検査で十分である。[192]
オックスフォード大学のエビデンスに基づく医療センター(CEBM)は、「隔離後または退院後にRT-PCRで再検査で陽性となった「新たな」軽症患者や人々のかなりの割合は感染性がなく、免疫系が効率的に処理した無害なウイルス粒子を単に排除しているだけである」という証拠が積み重なっていることを指摘し[194][195]、「検査を標準化し、定期的に較正するための国際的な取り組み」を求めている。[ 176] 9月7日、英国政府は「陽性検査結果の予測値が低下する、感染率の低い時期にSARS-CoV-2 RNA検査結果が陽性であることを保証するために、研究室で実施すべき手順に関するガイドライン」を発表した。[196]
2021年1月4日、米国FDAは、Curative SARS-Cov-2 AssayリアルタイムRT-PCR検査における偽結果、特に偽陰性結果のリスクについて警告を発した。[49]
等温核酸増幅検査
ある研究では、ID Now COVID-19検査の感度は85.2%だったと報告されています。アボットは、この問題は分析の遅れが原因かもしれないと回答しました。[197]別の研究では、この感度の低さを理由に臨床現場での検査を拒否しました。[198]
確認テスト
WHOは、検査能力のない国やCOVID-19に関する経験が限られている国立研究所に対して、最初の5つの陽性サンプルと最初の10の陰性サンプルを16のWHO参照研究所のいずれかに確認検査のために送ることを推奨している。[199] [200] 16の参照研究所のうち、7つはアジア、5つはヨーロッパ、2つはアフリカ、1つは北米、1つはオーストラリアにある。[201]
国や地域の対応
アイスランド
アイスランドは、積極的な接触者追跡、入国制限、検査、隔離によってパンデミックを管理したが、ロックダウンはそれほど積極的ではなかった。[202]
インド
イタリア
研究者らは、イタリアで最初のCOVID-19による死者が出たヴォーの全住民を検査した。彼らは約3,400人を10日間隔で2回検査した。陽性反応を示した人の約半数は無症状だった。発見された症例はすべて隔離された。コミューンへの移動を制限するとともに、新規感染は排除された。[203]
日本
日本は他のアジア諸国とは異なり、 SARSやMERSのパンデミックを経験しなかったため、PCR検査体制が十分に整備されていなかった。[204] [ 205]日本は当初、重症患者とその濃厚接触者を優先的に検査した。日本の新型コロナウイルス専門家会議は、感染クラスターを特定するためにクラスター対策を選択した。[204] [205]専門家会議は、武漢での発生を分析し、クラスターにつながる条件(密閉空間、密集空間、密接接触)を特定し、それらを避けるよう求めた。[205] [206]
1月に最初の感染者が発見されてからすぐに接触者追跡者が行動を起こした。当初は行政検査のみが行われていたが、3月6日にPCR検査が保険適用となった。民間企業も検査を始め、検査体制は徐々に拡大していった。[204] [207]
4月3日には、陽性反応が出た人でも無症状や軽症であれば自宅やホテルで療養することが法的に認められ、病床不足は解消された。[208]第一波(中国発)は封じ込められたが[209] 、 3月中旬の第二波(欧州や米国からの帰国者によるもの)により、4月には感染が拡大した。[205] 4月7日、日本は緊急事態宣言を発令した(都市封鎖や外出制限を行わなかったため、ロックダウンほど厳格ではない)。[205] [208] [210] 5月13日には抗原検査キットが保険適用となり、PCR検査と組み合わせて診断に用いられるようになった。[211] [212]
日本は陽性率は低いものの、一人当たりのPCR検査数は他の国に比べてはるかに少ないままで、3月には超過死亡が観測された。[206] [検証失敗] [210] [検証失敗] [213]専門家会議は、「日本の医療システムはもともと肺炎サーベイランスを行っており、肺炎を発症する重症患者のほとんどを検出できる。日本には多数のCTスキャナーがあり、全国の小規模病院に普及しているため、肺炎患者を見逃すことはほとんどない。その意味では、PCR検査を中心に実施している他の国々と同じ基準を満たしている」と述べた。[206] [213]グループは、診断にはCTスキャンデータと医師の所見を使用することを推奨した。 [214] [215]ダイヤモンドプリンセスクルーズ船では、最初は陰性だった人が後に陽性と判明した人が多かった。軽症または無症状であったコロナウイルス陽性者の半数はCTスキャンで肺炎の所見があり、CT画像には感染の特徴であるすりガラスの影が見られました。[214] [216]
日本の1日のPCR検査能力は7月18日時点で約3万2千件で、4月時点の1万件の3倍以上。これに抗原検査を加えると約5万8千件となる。人口1000人あたりの検査数はアメリカが日本の約27倍、イギリスが20倍、イタリアが8倍、韓国が2倍となっている(7月26日現在)。[217] [218] [219] 7月に入ってコロナウイルスの感染者数や入院患者数は増えているが、重症者数は増えていない。これは4月に比べて7月の感染者に対する検査がきちんと行われたためだと考えられる。4月は感染者数の増加に検査数が追いつかず、検査基準が厳しかったため、ピーク時には検査陽性率が30%を超えた。つまり、感染者に対してPCR検査を受けていないケースがかなりあったということだ。第一波では若年層を中心に軽症者や無症状感染者が多かったが、重症者を優先的に検査したと考えられる。つまり、検査体制を強化したことで、以前よりも感染の実態をかなり把握できるようになったということだ。[220] 7月末には軽症者や無症状感染者のための宿泊施設が満杯となり、当局は病院に軽症者用のベッドを用意するよう要請した。しかし、軽症者による病床占拠により、他疾患の患者の治療や職員を含めたICU体制の維持が困難になった。[221] [222] [223]
ロシア
ロシアは2020年4月に300万人を検査し、18万3000人が陽性反応を示した。[224] 4月28日、ロシア連邦保健医療監視局(Roszdravnadzor)のアンナ・ポポワ局長は、506の研究所が検査を実施しており、陽性反応を示した人の45%は無症状であり、患者の5%は重症であり、感染者の40%は家族からの感染であると述べた。症状が現れてから6日後から1日後には病状が改善した。モスクワの医師3200人を対象に抗体検査を実施し、20%に免疫が見つかった。[225]
シンガポール
接触者追跡、入国制限、検査、隔離により、シンガポールは完全なロックダウンを行わずに初期の感染拡大を食い止めた。[226]
スロバキア
2020年10月、スロバキアは週末に人口540万人のうち362万人を検査し、総人口の67%(成人人口の82%)を占め、38,359人が陽性と判定され、検査を受けた人の1.06%を占めた。政府は、この大規模検査がウイルスの制御とロックダウンの回避に大きく役立つと考えており、後日再度実施する可能性がある。[227]
韓国
韓国の広範な検査アプローチは感染拡大の抑制に役立った。韓国政府は2000年代初頭に数年かけて、主に民間の研究所で検査能力を構築した。[228]
政府は住民登録番号制度を悪用した。当局は兵役資格のある若者を社会奉仕員、治安・公衆衛生医師として動員した。公衆衛生医師は主に軽症患者が収容されている保健所や生活治療センターに派遣され、PCR検査を実施し、軽症患者の管理を行った。社会奉仕員は人員不足を補うため薬局で働いた。韓国の100万人当たりのPCR検査数は4月13日時点で世界最高で、6月中旬には2万件にまで増加した。3月には韓国企業27社が4,860万ドル相当の検査キットを輸出し、120カ国以上から検査キットや人道支援の提供を要請された。韓国当局は、より重症の患者のために病床を空けるため、無症状や軽症の患者を1つの施設で隔離・管理する治療センターを設置した。
センターは主に国立施設や企業の研修センターに設置された。2015年5月の韓国のMERS隔離の失敗により、韓国はパンデミックに直面しなかった国々よりもCOVID-19に対する備えができていた。当時の朴槿恵大統領は2016年に韓国CDC認可の民間部門による感染症検査を許可した。韓国にはすでに感染症患者を他の患者とは別に隔離、検査、治療するシステムがあった。呼吸器疾患だが疫学的関連性のない患者は国立病院で治療され、疫学的関連性のある患者は選ばれた診療所で治療された。[229] [230] [231] [232] [233] [234] [235] [236] [237]
韓国は大規模なドライブスルー/ウォークスルー検査プログラムを確立した。しかし、最も一般的な方法は「移動検査」であった。大邱市では、3月23日までに54%のサンプルが自宅または病院で採取された。戸別訪問によるサンプル採取は、感染の可能性がある患者の移動リスクを回避したが、追加のスタッフが必要だった。韓国は、2,700人以上の公衆衛生医師を動員することでこの問題を解決した。[229] [233] [232]

2021年1月、デンマークがUAE便を5日間停止したことで、UAEのCOVID-19検査結果が精査された。このヨーロッパ諸国は、湾岸諸国で行われている検査プロセスに不正がある疑いが高まっているため、UAEからのフライトを禁止したと述べた。デンマークのベニー・エンゲルブレヒト運輸大臣は、 UAEからの旅行者の陰性検査が適切に実施された実際のスクリーニングであることを確認するために時間をかけていると述べた。[247]

国別の検査統計
検査戦略は国によって、また時期によっても異なり、[255]非常に広範囲に検査を行っている国もあれば[8]、重症者のみの検査に絞っている国もある。[6]症状のある人だけを検査する国は、症状のない人も検査する国よりも「確定者」/「検査済み」の数字が高くなります。[256] 2つの国が、検査する対象者を含め、あらゆる点で同じであれば、より多くの人を検査する国の方が「確定者 / 人口」が高くなります。研究によると、死亡者数に比べて検査数が多い国では、推定致死率が低く[9]、症例の年齢分布が若いこともわかっています。[11]
