神経眼科は、神経学と眼科学の分野を融合させた専門分野であり、視覚系に症状が現れる複雑な全身疾患を扱うことが多い。神経眼科医は、まず神経学または眼科学のいずれかで研修医課程を修了し、その後、神経眼科のフェローシップを受ける。重篤な神経眼疾患の患者では診断検査が正常となる場合があるため、[ 1 ]詳細な病歴聴取と身体診察が不可欠である。
神経眼科医に紹介される病理には、求心性視覚系障害(例:視神経炎、視神経症、乳頭浮腫、脳腫瘍または脳卒中)と遠心性視覚系障害(例:瞳孔不同、複視、眼筋麻痺、眼瞼下垂、眼振、眼瞼痙攣、眼または眼筋の痙攣、片側顔面痙攣)が含まれます。神経眼科医の最大の国際学会は北米神経眼科学会(NANOS)[ 2 ]で、年次総会を主催し、Journal of Neuro-Ophthalmologyを発行しています。神経眼科医は、多くの場合、大規模な大学病院の教員です。患者は、他の分野(リウマチ学、内分泌学、腫瘍学、心臓病学)で併存疾患を抱えていることがよくあります。
Neuro-ophthalmology focuses on diseases of the nervous system that affect vision, control of eye movements, or pupillary reflexes. Neuro-ophthalmologists often see patients with complex multi-system disease and unusual diagnosis. Neuro-ophthalmologists are often active teachers in their academic institution, and the first four winners of the prestigious Straatsma American Academy of Ophthalmology teaching awards were neuro-ophthalmologists.[3] Neuro-ophthalmology is mostly non-procedural, however, neuro-ophthalmologists may be trained to perform eye muscle surgery to treat adult strabismus, optic nerve fenestration for idiopathic intracranial hypertension, and botulinum injections for blepharospasm or hemifacial spasm.[4]
At the turn of the 20th century, there was no textbook in the English language on neuro-ophthalmology. In 1906, Dr. William Campbell Posey, Professor of Ophthalmology in the Philadelphia Polyclinic and Dr. William G. Spiller, Professor of Neurology in the University of Pennsylvania, edited The Eye and Nervous System: Their Diagnostic Relations By Various AuthorsJ.B. Lippincott & Co. According to the Preface to this book, "Although it is generally conceded that a knowledge of neurology is indispensable to the ophthalmologist and that an acquaintanceship with ophthalmology is of the greatest service to the neurologist, there is no book in the English language which covers the ground where the two specialties meet."[5]
Frank B. Walsh was a pioneer of neuro-ophthalmology, helping to popularize and develop the field. Walsh was born in Oxbow, Saskatchewan in 1895 and earned a degree from University of Manitoba in 1921. He joined the Wilmer Ophthalmological Institute at Johns Hopkins University and began organizing Saturday morning neuro-ophthalmology conferences. Walsh compiled the first neuro-ophthalmology textbook, which was published in 1947 and has been updated over the years by generations of his students.[6]
機能的神経画像診断の改善により、多くの神経疾患および神経眼科疾患の理解、評価、管理が向上しています。神経科学の理解が進むにつれて、神経眼科医は診断だけでなく治療にもますます長けてきており、従来は視力に深刻な影響を与えていた疾患を治療するための新しい治療法が登場しています。[ 7 ]例えば、遺伝子治療を用いてレーバー遺伝性視神経症を治療する臨床試験が2014年2月に開始されました。[ 8 ]これは中枢神経系における遺伝子治療の最初の使用例の1つです。網膜神経節細胞の再生の理解や、視神経から脳へのシナプス結合の再構築においても、中枢神経系の他の領域よりも進歩が見られます。[ 1 ] [ 9 ] [ 10 ]国立衛生研究所の目標の1つは、視覚系を中枢神経系の神経可塑性と再生医療を理解するための窓として利用することであり、[ 11 ]神経科学の分野では将来有望であり、神経眼科学と密接に結びついています。
